Describes the process of score reporting for the Rhode Island nursing assistant certification exam

Exam Results

You will get a notification email from Credentia Platform when a new exam score has been posted to your online account. To see your score report, please login to your Credentia account by clicking the “Login” button on the top of the Rhode Island nurse aide website page at https://credentia.com/test-takers/ri/

Score reports are generally available within a few hours after the day’s testing event is completed. If it has been more than twenty-four (24) hours and you’re not able to see your score report in the Credentia Platform, please contact customer service at 888-204-6249.

Skills Evaluation

The Nurse Aide Evaluator will also fax your Skills Evaluation results for scoring. After the Nurse Aide Evaluator evaluates your performance, he or she will fax the Skills Evaluation answer sheet for scoring. The Score Report will indicate whether you have passed or failed the Skills Evaluation.

Occasionally, due to infrequent technical difficulties, Score Reports may not be available on the day of testing. If this happens your answer sheet will be mailed overnight to Credentia for hand scoring. Your Score Report will then be available to you (via your Credentia Platform account) within 5-7 calendar days after testing. For questions regarding delayed Score Reports, please call 888-204-6249.

Failing

If you fail the Written (or Oral) Examination or the Skills Evaluation, your Score Report will provide you with information on how to re-take either or both parts. A new examination fee is required each time you re-take any part of the NNAAP Examination. If you fail the Written (or Oral) Examination or the Skills Evaluation, your Score Report will provide you with information on re-taking either or both parts. State and federal regulations allow you three (3) attempts to pass both the Skills Evaluation and the Written (or Oral) Examination. If you should fail either part or both parts three (3) times, you will be required to success fully complete a state-approved nursing assistant training program and re-take both parts. You must take and pass both the Written (or Oral) Examination and the Skills Evaluation within a twelve (12) month period in order to be placed on the Rhode Island Nursing Assistant Registry.

If you Receive a Failing Score

  • If you fail the Written (or Oral) Examination or the Skills Evaluation, your Score Report will tell you how to retake either or both parts. 
  • If you fail either part of the NNAAP Examination three (3) times, you will need to complete another training program and retake BOTH parts of the NNAAP Examination, not just the part that was failed. 
  • In addition, if you were scheduled to take the exam three (3) times and did not become licensed — either as a result of failing the exam or absence(s) — you will need to retrain. 
  • If you are either an E3 or an E4 candidate, you have ONLY ONE attempt to pass the examination. If you fail, you MUST retrain and reapply under eligibility route E0.

Passing

Once you have passed both the Written (or Oral) Examination and the Skills Evaluation, you may verify your Nursing Assistant license status with the Rhode Island Department of Health at https://health.ri.gov/. The Rhode Island Department of Health will mail your Nursing Assistant license to you. If you have not received your Nursing Assistant license within sixty (60) days of your examination date, contact the Rhode Island Department of Health at (401) 222-5888.
